Arthroscopic debridement of penetrating knee joint injuries has become a common treatment method. A comparative study was undertaken to compare this method with open joint debridement. Fourteen penetrating knee joint injuries (fourteen patients) were treated by arthroscopic examination and debridement and were compared to sixteen penetrating knee joint injuries (fifteen patients) treated by open debridement. There were no resultant infections or operative complications in either group. Of note, the arthroscopic debridement group had a shorter postoperative hospital stay [mean of 1.6 days compared to a mean of 2.6 days in the open debridement group (p < 0.02)], a significant incidence of additional intra-articular injuries detected (p < 0.01), less postoperative pain, and a superior cosmetic result. We conclude that arthroscopic debridement of penetrating knee joint injuries is a safe and effective method of treatment, providing additional diagnostic information while minimizing morbidity and reducing hospital stay.

The synovium is a loose connective tissue that separates the intra-articular (IA) joint compartments of all diarthrodial joints from the systemic circulation. It can be divided into two layers: the intima, a thin and cell-dense layer atop a more heterogeneous subintima, composed of collagen and various cell types. The subintima contains penetrating capillaries and lymphatic vessels that rapidly clear injected drugs from the joint space which may vary not only with drug size and charge but also with the microstructure and composition of the intima and subintima of the synovium.

This study aimed to explore the construction of experimental animal models replicating cartilage defects across diverse load-bearing sites, compare self-repair conditions, and examine the role of mechanical stimulation in cartilage self-repair. Experimental animal models were established in rabbits to simulate full-thickness cartilage defects without penetrating the subchondral bone, at various load-bearing sites, including the posterior femoral condyle, anterior femoral condyle and femoral trochlear of knee joint, and the humerus of the shoulder joint. The successful exposure and construction of cartilage defects at the anterior femoral condyle, femoral trochlear, and posterior femoral condyle through the medial extension of surgical incision.
