Introduction: Necrotizing soft tissue infections (NSTI) are increasing, posing a significant risk of morbidity and mortality. Due to nonspecific symptoms, a high index of suspicion is crucial. Treatment involves a multidisciplinary approach, with broad-spectrum antibiotics, early surgical debridement, and life support.

Pyomyositis is an infective involvement of systemic skeletal muscles. We discuss the case of 43-year-old male who presented with quadriparesis, anemia, and hypercalcemia, leading to suspicion of multiple myeloma, and on FDG PET-CT, incidentally, pyomyositis was found. FDG PET-CT thus helped in diagnosing an occult infection which helped in the treatment of the patient.

Pyomyositis is a rare disease in temperate climates. This case is a unique presentation of pyomyositis of the rectus abdominal muscle mimicking an acute abdominal process. Most reported cases of pyomyositis are of extremity infections with Staphylococcus aureus.

Unlabelled: Thigh abscesses due to pyomyositis are uncommon. To guide empiric antibiotic therapy in diabetics we determined the rate of such infections due to oxacillin-resistant Staphylococcus aureus and Gram-negative organism infections, and whether the occurrence of oxacillin-resistant pathogens increased during the study period. We retrospectively reviewed 39 adult patients with diabetes mellitus treated for a deep thigh abscess.
