The characteristic clinical picture of the McKusick-Kaufman syndrome was observed in a girl of Belorussian background. A supernumerary nipple was the only finding not previously described in reported familial cases of this syndrome. In general, the range of phenotypic variability should be the same in both familial and sporadic cases. For this reason we feel that some sporadic cases reported as patients with unusual variants of the McKusick-Kaufman syndrome have more likely "new" genetic syndromes or non-genetic conditions, which only resemble the syndrome. The comparison of the phenotypes of alleged sporadic cases with familial cases of single gene syndromes should be helpful in syndrome delineation, if the number of familial observations is sufficient.

Rocky Mountain spotted fever (RMSF) kills people and dogs in rural communities throughout the Americas and in urban epidemics in Brazil and Mexico. The companion Currents in One Health by Foley et al, AJVR , March 2025, addresses the urban ecology of this devastating disease across the Americas. Cases acquired from Dermacentor spp ticks are sylvatic and sporadic, in contrast with peridomestic cases from Rhipicephalus sanguineus sensu lato, which relies on dogs as hosts.

Anti-NMDA (N-methyl-D-aspartate) receptor encephalitis (ANRE) is a rare autoimmune condition targeting brain receptors, often linked to ovarian tumors in young women. In severe cases, it can lead to status epilepticus, but in sporadic cases, it may progress to super-refractory status epilepticus (SRSE), a dangerous state of continuous or repetitive seizures demanding urgent medical attention that continues or recurs more than 24 hours after the initiation of anesthetic therapy. We present a case report of anti-NMDA receptor limbic encephalitis-triggered SRSE terminated with vagus nerve stimulation (VNS) and titrated to high stimulation parameters in the immediate postoperative period.
