Historically, oxidants have been considered mechanisms of glomerulonephritis, but a direct cause-effect correlation has never been demonstrated. Several findings in the experimental model of autoimmune conditions with renal manifestations point to the up-regulation of an oxidant/anti-oxidant system after the initial deposition of autoantibodies in glomeruli. Traces of oxidants in glomeruli cannot be directly measured for their rapid metabolism, while indirect proof of their implications is derived from the observation that Superoxide Oxidase 2 (SOD2) is generated by podocytes after autoimmune stress.

The APLAR has published a set of recommendations on the management of systemic lupus erythematosus (SLE) in 2021. The current consensus paper supplements and updates specifically the treatment of lupus nephritis (LN) according to two rounds of Delphi exercise from members of the APLAR SLE special interest group, invited nephrologists, histopathologists, and lupus nephritis patients. For initial treatment of LN, we recommend a combination of glucocorticoids (GCs) with cyclophosphamide (CYC), mycophenolate mofetil (MMF), or the calcineurin inhibitors (CNIs) as first-line options.

- This study tested rituximab (RTX) on patients with primary membranous nephropathy (PMN) who were dependent on calmodulin inhibitors (CNIs) and not fully in remission.
- Among 36 patients, those who were drug-dependent showed a significant reduction in proteinuria and improved remission rates after 12 months of RTX therapy, with complete remission increasing from 10% to 70%.
- The results indicated that RTX not only helped patients overcome CNI dependency but also led to significant immunological improvements while minimizing the risks of adverse effects.
