Neurofibromatosis can involve the mediastinum. A 44-year-old woman with a dumbbell-shaped mediastinal mass developed a large pleural effusion, respiratory failure and fatal hemoptysis. Autopsy revealed systemic neurofibromatosis involving the mediastinum and pleura. Mediastinal and pleural hemorrhage probably occurred as a result of an eroded thoracic artery. Massive hemorrhage in mediastinal neurofibromatosis occurs uncommonly but with potentially fatal results.

Background: Kirschner wires or pins were widely used for osteosynthesis in trauma surgery. Breakage of osteosynthesis material and intra-thoracic migration is a complication that has occasionally been described. We reviewed the literature to study the frequency and pathophysiology of such migrations.

Tension gastrothorax is a rare and potentially fatal complication of a diaphragmatic hernia, in which a massively dilated, intrathoracic stomach compresses the lungs and mediastinum, causing cardiorespiratory compromise, in a mechanism akin to tension pneumothorax. Although it is very rare, tension gastrothorax has been reported in the literature; however, such reports are almost exclusively restricted to its clinical presentation and treatment in emergency departments. To the best of our knowledge, no adult autopsy case reports of tension gastrothorax have been reported in the literature.

Background: Intrathoracic migration of a ventriculoperitoneal shunt (VPS) is a phenomenally rare complication, with the supradiaphragmatic intercostal variant even more so. Whereas it can prove debilitating or even fatal via massive hydrothorax, the causative mechanism and proper management of this occurrence are undefined.

Case Presentation: A 17-month-old girl who had undergone VPS insertion at one month of age was brought to our department for somnolence and dyspnea, which had a sudden onset.

Pleural mesothelioma (PM) is a fatal disease with limited treatment options. Recently, PM management has improved with the development of immune checkpoint inhibitors (ICIs). In first-line therapy, dual PD-1 and CTLA-4 blockade enhances tumor control and patient survival compared with chemotherapy.

Article Synopsis
  • Blunt intrathoracic tracheal injuries are uncommon but can be life-threatening, necessitating prompt surgical intervention for better patient outcomes.
  • Diagnosing these injuries is challenging due to vague symptoms and sometimes inconclusive imaging results; heightened suspicion is warranted in cases of respiratory distress post-trauma.
  • The case report highlights a patient with such injuries, emphasizing that early surgical repair can improve airway integrity and decrease the chances of fatality.
