This case report describes the diagnosis, management and the short- and long-term outcomes of a dog with paraprostatic cysts and associated perineal herniation. A 9-year-old male entire Bearded Collie was referred for a caudal abdominal mass and right perineal hernia noted on routine examination. Computed Tomography (CT) imaging of the abdomen demonstrated a large bilobed fluid filled structure adjacent to the prostate, extending into the peritoneal cavity and the right perineal space, causing perineal herniation and partial obstruction of the right ureter.

Two intact male dogs were evaluated for symptoms, including hematuria, prostatomegaly, anuria, lethargy, and abdominal mass. Presurgical evaluations, including complete physical examinations, blood examinations, abdominal radiography with contrast (only in Case 2), ultrasonography, and computed tomography and magnetic resonance imaging (only in Case 1), were performed. A paraprostatic cyst was diagnosed initially, and laparoscopic exploration and surgery were performed.

The etiology of canine perineal hernia (PH) remains unclear, although as a disease of older male dogs, it is likely to be hormonal. The role of the prostate in the formation of PH has been questioned; however, prospective and systematic evaluation of prostates in these dogs is absent in the literature. In this prospective case-control study, CT imaging was used to assess prostatic changes in dogs with PH (n = 46) and compare these findings with those of intact age-matched male dogs (n = 23).

A 7-year-old male Caucasian shepherd presented with a 3 month history of intermittent hematuria, penile discharge, and abdominal pain and distension. The dog had a history of prostatic hyperplasia with multiple cysts, diagnosed by the referring clinician two years prior to the case presentation. Two oral courses of antibiotics and antiandrogens were administered by the treating veterinarian without resolution.
