44 patients with destructive pulmonary tuberculosis received chemotherapy (intermittent antibacterial drugs, i.v. isoniazid) and artificial pneumothopax (AP). 42 patients had infiltrative disease (destructive in 15, disseminated in 27 cases). Discharge of the bacilli occurred in 95.4%. AP application was followed by thoracocautery in 3-4 weeks to form appropriate collapse of the affected lung portions. Short adhesions were burnt, plane ones were extrapleurally detached according to the technique developed in the Novosibirsk Research Institute of Tuberculosis. A complete correction of pneumothorax was achieved in 43 patients. Neither intra- nor postoperative complications were reported. The discharge stopped in 100%, the caverns closed in 93.1% of the cases.

Pulmonary fibrosis (PF) is a high-mortality lung disease with limited treatment options, highlighting the need for new therapies. Cyclin-dependent kinase 8 (CDK8) is a promising target due to its role in regulating transcription via the TGF-β/Smad pathway, though CDK8 inhibitors have not been thoroughly studied for PF. This study aims to evaluate the potential of E966-0530-45418, a novel CDK8 inhibitor, in mitigating PF progression and explores its underlying mechanisms.

Streptococcus pneumoniae (Sp; pneumococcus), the most common agent of community-acquired pneumonia, can spread systemically, particularly in the elderly, highlighting the need for adjunctive therapies. The airway epithelial barrier defends against bacteremia and is dependent upon apical junctional complex (AJC) proteins such as E-cadherin. After mouse lung challenge, pneumolysin (PLY), a key Sp virulence factor, stimulates epithelial secretion of an inflammatory eicosanoid, triggering the infiltration of polymorphonuclear leukocytes (PMNs) that secrete high levels of neutrophil elastase (NE), thus promoting epithelial damage and systemic infection.

A 30-year-old man with a 5-month history of lumbar pain was hospitalized due to worsening pain and restricted mobility. CT and MRI scans indicated multiple vertebral destructions, prompting further assessment with 18F-FDG PET/CT. PET/CT imaging showed widespread and avid 18F-FDG uptake in the bones, mainly in the vertebrae.

However, the optimal strategy to combine EGFR tyrosine kinase inhibitors (TKIs) with radiotherapy for maximum efficacy and minimal toxicity is still uncertain. Notably, EVs, which serve as communication mediators among tumor cells, play a crucial role in the anti-tumor immune response.
