Background And Objectives: The middle fossa approaches are tremendously versatile for treating small vestibular schwannomas, selected petroclival meningiomas, midbasilar trunk aneurysms, and lesions of the petrous bone. Our aim was to localize the internal acoustic canal and safely drill the petrous apex with these approaches. This study demonstrates a new method to locate the internal acoustic canal during surgery in the middle fossa.

Comparison of Bupivacaine plus Dexmedetomidine versus Bupivacaine plus Fentanyl for Caudal Block in Pediatric Infraumbilical Surgery.

Mymensingh Med J

January 2025

Dr Md Khairul Kabir Khan, Junior Consultant, Department of Anaesthesiology and Intensive Care Unit, Mymensingh Medical College Hospital, Mymensingh, Bangladesh; E-mail:

Different additives have been used to improve the duration and quality of analgesia of the local anaesthetic used in the single-dose caudal block technique, such as opioids, epinephrine, clonidine, neostigmine, etc. Dexmedetomidine is a potent and a highly selective α2-adrenergic agonist having a sympatholytic, sedative, and analgesic effect and has been described as a safe and effective additive in many anaesthetic and analgesic techniques. Another agent is Fentanyl, a lipophilic opioid, is added frequently to local anaesthetics which least likely to cause respiratory depression when given extradurally, because of its high lipid solubility.

Petroclival approaches remain challenging given abundant cranial nerves and vessels. Common trajectories include transsphenoidal, transoral, middle fossa-extradural, and posterior through the cerebellar peduncle. We report a unique intra-axial, intradural approach to the petroclival and cavernous sinus.

Article Synopsis
  • Facets synovial cysts (FSCs) are benign growths from facet joints that can lead to nerve pain, and this study explored the effectiveness of CT-guided procedures for their treatment, comparing IPCR and DF methods.
  • The retrospective review examined 90 FSC rupture attempts on 75 patients over 10 years, revealing that IPCR alone had a 70% success rate, but when combined with DF after IPCR failure, the success rate rose to 90.6%.
  • The study concluded that having the option to use DF after IPCR failure significantly improves treatment outcomes, highlighting the advantages of using CT guidance for these procedures.

Cranioplasty to restore calvarial defects involves reconstruction with alloplastic materials or autologous tissues in order to provide the best protection to all intracranial contents. Sometimes, autologous bone may not be available; therefore, different materials have emerged such as polymethylmethacrylate plate, titanium mesh, and hydroxyapatite. However, when it is impossible to replace the autologous bone, the aesthetic result is generally unsatisfactory.
