This report describes one complication related to retrograde positioning of a catheter in the jugular vein in a patient in a coma resulting from subarachnoid haemorrhage. The catheter was found in the cervical subarachnoid space, as confirmed by radiography with contrast medium. Attention is focused on the fact that this technique, usually performed easily and safely, may occasionally present potentially severe complications.

Methods: A retrospective study was conducted in 201 patients. Thirty-day follow-up data was analyzed to compare the outcomes of different techniques and evaluation of risk factors.

Lemierre's syndrome is a rare and potentially life-threatening complication of head and neck infections, such as bacterial pharyngitis or tonsillitis. It is characterized by the extension of infection into the lateral pharyngeal spaces, leading to subsequent septic thrombophlebitis of the internal jugular vein(s). Although relatively uncommon since the advent of appropriate antibiotic therapy, the incidence of Lemierre's syndrome has increased in the past 15 years, especially among young, healthy adults.

Objective(s): Previous literature suggested the advantage of combined short-long axis (CSLA) technique to avoid posterior wall puncture during internal jugular vein (IJV) cannulation. The purpose of this study is to define the best ultrasound-guided IJV cannulation technique regarding the success rate in the first trial of insertion, time to successful central line placement, number of attempts, procedural complications, and operator satisfaction.
