A case of a 38-year male patient treated for severe Wegener's granuloma has been presented. Immunosuppressive therapy including cyclophosphamide and prednisone produced a 4-week improvement followed by relapsed with advanced organ involvement (severe cardio-respiratory and renal failure). Intensive therapy with cyclosporine A and prednisone (given intravenously and next orally) carried out for 4 months produced a favourable response with complete regression of organ involvement. Actually, the patient is living in a complete remission since 4 years, and takes no drugs since 1 year.

Severe pulmonary arterial hypertension and cardiogenic shock in acute systemic lupus erythematosus.

We describe a woman in her late 20s with newly diagnosed systemic lupus erythematosus (SLE), who presented with fulminant pulmonary arterial hypertension (PAH) requiring inotropic and extracorporeal support. She was established on triple pulmonary vasodilator therapy with concurrent aggressive immunosuppression; however, treatment was complicated by infection and diffuse alveolar haemorrhage, necessitating delays in immunosuppression and withdrawal of epoprostenol. Despite this, with ongoing suppression of her SLE, her pulmonary haemodynamics improved, with normal pressures on right heart catheterisation several months later allowing stepdown to sildenafil monotherapy.

Post-transplant lymphoproliferative disorder (PTLD) poses a serious challenge in kidney transplant recipients. Epstein-Barr virus (EBV)-seronegative recipients have a significantly increased risk of PTLD, but few studies have investigated risk factors for PTLD in EBV-seronegative recipients in the current era of immunosuppression. This cohort study from Norway and western Denmark included first-time kidney transplant recipients between 2007-2021, and estimated the cumulative incidence, risk and prognosis of PTLD.
