Stentless valves may be an alternative to conventional stented bioprostheses and homografts. From all tissue valves homografts proved to have the best long term results. But homografts are not always available. Stentless bioprostheses are similar to homografts concerning in implantation technique, valve anatomy and physiologic function. The stentless design leads to remodeling of the left ventricle and the aortic root, resulting in a superior hemodynamic profile. Nevertheless long term results are required.

The Ross procedure continues to be the best procedure to address unrepairable aortic valve pathology, especially in young adults. The Achilles heel of this procedure has been aortic root dilation and the potential need for a reoperation that may be associated with slightly increased risks in addition to the need for intervention on the pulmonary outflow tract. Modifying the Ross procedure by autograft inclusion inside a Dacron graft seems to have the potential advantage of stabilizing the autograft diameter, which may be associated with improved durability and decrease the need for future intervention.

Objectives: Aortic valved allografts (homografts) have been used alternatively to mechanical or biological valve prostheses in expectation of better durability; however, homograft valves do degenerate, and redo procedures have proven challenging due to heavy wall calcification. The aim of the study was to compare the outcome of open surgical (SAVR) and transcatheter aortic valve replacement (TAVR) in degenerated homografts.
Methods: Between 1993 and 2022, 81 patients underwent repeat aortic valve procedures having previously received an aortic homograft.
