We present 2 cases of caseous mitral annulus calcification (MAC) in which one patient was asymptomatic whereas the second experienced left hemianopsia. Both patients underwent transthoracic and transesophageal echocardiography exams which revealed severe MAC with a mass consistent with caseous MAC. A mobile component of the caseous MAC was observed in the patient with left hemianopsia.

An 83-year-old man with known history of atrial fibrillation presented for preoperative evaluation for elective left nephrectomy for cancer. Transthoracic echocardiogram revealed a large, free-floating, left atrial mass. Further profiling with transesophageal echocardiogram showed a free-floating mass intermittently obstructing the mitral valve.

A 72-year-old woman underwent left bundle branch area pacing, and subsequent transthoracic echocardiography demonstrated potential septal lead perforation. Transesophageal echocardiography revealed an intracardiac mass, which resolved with anticoagulation. This case highlights left ventricular thrombus as a potential complication of septal lead perforation.

A 77-year-old man with multiple comorbidities presented with cough, dyspnea and nonspecific malaise. Chest computed tomography revealed a mass in the right lower lobe of the lung, along with an associated hypodense lesion in the left atrium. Echocardiography showed a mobile mass in the left atrium, initially suspected to be a thrombus.

Objective: To identify the risk factors associated with moderate to severe perivalvular leakage (PVL) after transcatheter aortic valve replacement (TAVR) and to construct a prediction model for this risk.
Methods: A retrospective analysis was conducted on 128 patients with severe aortic stenosis who had received TAVR in The Second Hospital of Hebei Medical University from January 2019 to January 2024. The length of the aortic regurgitation bundle and annular circumference ratio were measured by transesophageal echocardiography immediately after the valve implantation.
