A sixty-year-old man was admitted to our hospital due to prolonged left anterior chest oppression and hypotention. The electrocardiogram revealed acute inferior myocardial infarction, confirmed by the coronary angiography which showed occluded right coronary artery (RCA) at the segment 4AV. The intra-coronary pro-urokinase infusion and coronary angioplasty successfully revascularized the occluded 4AV segment. Although the 4AV segment perfused small area, he developed severe pulmonary edema on the second hospital day. A transesophageal echocardiography revealed severe mitral regurgitation due to posterior mitral valve prolapse by the ruptured tendineae with a mobile mass. The damaged mitral valve was replaced by the prosthetic mechanical valve (SJM 25M), resulted in stable hemodynamic state. He discharged one month after the operation.

Objective: The aim of this study was to determine if oral beta-lactam therapy is non-inferior to alternative therapy at discharge following inpatient treatment with an IV cephalosporin for acute pyelonephritis.
Design: Institutional Review Board (IRB)-approved, multicenter, retrospective, non-inferiority cohort (15% non-inferiority margin).
Setting: Six hospitals within two healthcare systems.
Familial Platelet Disorder with associated Myeloid Malignancy (FPDMM, FPD/AML, -FPD), caused by monoallelic deleterious germline variants, is characterized by bleeding diathesis and predisposition for hematologic malignancies, particularly myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML). Clinical data on FPDMM-associated AML (FPDMM-AML) are limited, complicating evidence-based clinical decision-making. Here, we present retrospective genetic and clinical data of the largest cohort of FPDMM patients reported to date.

Bilastine is a non-sedating antihistamine, a preferred option over sedating antihistamines. Despite the success of the CD19xCD3 T cell engager blinatumomab in B-cell acute lymphoblastic leukemia (B-ALL), treatment failure is common and can manifest with antigen loss and extramedullary disease (EMD) relapse. To understand the impact of leukemia genetics on outcomes, we reviewed 267 adult patients with B-ALL treated with blinatumomab and used next generation sequencing to identify molecular alterations. Patients received blinatumomab for relapsed/refractory (R/R) disease (n=150), minimal residual disease (MRD+) (n=88), upfront as induction (n=10), or as consolidation in MRD- state (n=19).
