Background: Gastric lipase is an important enzyme for dietary triglyceride digestion in normal subjects. Its regulation is unknown, as is the relation between the quantity and activity of the enzyme.
Methods: In a dose-response study we investigated the effect of low doses of pentagastrin (less than 1000 ng/kg/h) on the output of gastric lipase measured by a kinetic assay and an enzyme-linked immunosorbent assay (ELISA).
Results: In five healthy volunteers stepwise intravenous pentagastrin infusions of 0, 50, 100, 500, and 1000 ng/kg/h resulted in a stepwise increase in the lipase output, as measured with ELISA. However, the lipolytic activity, measured with a kinetic assay, decreased as the pH of the gastric contents decreased.
Conclusion: We conclude that secretion of the gastric lipase is stimulated by pentagastrin, but the simultaneous secretion of acid counteracts the lipolytic activity of the enzyme when food is not present.

Ten replicates of two broilers (14 d old) or one pig (50 d old) each were distributed into a 3 × 2 randomised factorial design with three pelleted diets (maize, barley or oat-based) and the two species. A 68-year-old woman with obesity and ulcerative colitis history, referred to the emergency department and transferred to the ICU, with severe epigastric belt-like abdominal pain (requiring opiates) with sudden onset 10 hours after the last meal. The clinical exploration presented painful abdomen with epigastrium peritoneal irritation. Laboratory tests showed elevated amylase (2600 U/l) and lipase (1127 U/l), with leukocytosis and neutrophilia.

Acute pancreatitis is characterized by the premature activation of pancreatic enzymes leading to autodigestion and inflammation, commonly caused by gallstones or chronic alcohol consumption. However, iatrogenic factors, such as migration of a percutaneous endoscopic gastrostomy (PEG) tube, can be a cause of acute pancreatitis but are less frequently reported in the literature. This is a case of a 77-year-old male patient with a medical history significant for myasthenia gravis with dysphagia requiring a PEG tube placement who presented with abdominal pain and elevated lipase levels.
