The treatment of perforated peptic ulcer, according to the experience of the Authors (327 cases), must be very pressing and always surgical. The closure of the perforation, the simplest treatment, seems the most suitable and the least dangerous for the survival of the patient, even though 68% of these patients will have to undergo surgery again in the course of 10 years. The radical treatment of resection, is reserved to few and carefully chosen cases, because it is loaded with sufficient risk. Acute bleeding peptic ulcer (86 cases) requires the operation: immediate or delayed according to the importance of the hemorrage: very serious, serious, moderate.

Methods: This study is a retrospective, observational, and comparative case series that included 25 consecutive highly myopic eyes with MHRD. All eyes underwent standard 23-gauge vitrectomy, inverted internal limiting membrane insertion into the macular hole, subretinal fluid drainage from an intentionally created extramacular retinal hole, and tamponade with either silicone oil (SO group, n = 13) or perfluoropropane (CF group, n = 12).

We report on a fatal case of invasive fungal rhinosinusitis with after lung transplantation. After endoscopic treatment and adjuvant medical therapy with isavuconazole, caspofungin and an investigational antifungal drug, there was a good clinical response with absence of endoscopic and radiographic disease. However, the patient developed disease recurrence, with signs of intracranial involvement on MRI, for which urgent endoscopic sinus surgery was performed and isavuconazole was restarted.
