Recent studies have reported a large gap between the forceful and assertive recommendations in the guidelines and real-world practice in the use of renin-angiotensin-aldosterone inhibitors (RAASi) therapies. A comprehensive, retrospective analysis of a large database of electronic medical records (>7 million patients) was undertaken to evaluate 3 pivotal concerns: (i) whether RAASi are being prescribed according to treatment guidelines, (ii) what happens to RAASi prescriptions after hyperkalemia events, and (iii) what the clinical outcomes are in patients whose RAASi are discontinued or who are prescribed at doses lower than the guidelines recommend. The results indicate that a substantial gap exists between guideline recommendations and real-world prescribing patterns for RAASi.

Background: Pills (tablets and capsules) are widely used to administer prescription drugs or to take supplements such as vitamins. Unfortunately, little is known about how much effort it takes Americans to swallow these various pills. More specifically, it is not known to what extent hard-to-swallow pills might affect treatment outcomes (eg, interfering with adherence to prescribed medications or causing clinical complications).

Tai ping sheng hui fang, the first medical formulary of the Song Dynasty, compiled from the 3(rd) year of Taiping-xingguo reign to the 3(rd) year of Chunhua reign (978 ∼ 992), under the edict of the Taizong Emperor, was popularly applied and extensively circulated and called "the first formulary of the Dynasty". It is extremely significant in the medical history of the Song Dynasty due to its theory of prescription art, practical prescriptions and clinical practice. During the process of its circulation, different versions appeared, including the Guozijian Orthodox Version, Guozijian Small-character Version, Chongwen Hall Abridged Version, Newly Carved Version of Zhuanyunsi, and the Local Abridged Version, thus adapting to the demands of various walks of life in the society.
