A newborn male developed diffuse myoclonus. Right frontal craniotomy revealed a thin hemispheric mantle and a cyst communicating with the right lateral ventricle. In the biopsy of the cyst wall there were the characteristic findings of malakoplakia, granulomatous inflammation with von Hansemann histiocytes and Michaelis-Gutmann bodies. The child died at 1 1/2 years of age.

Colonic malakoplakia is a rare chronic granulomatous disease that can involve many organs such as the lung, brain, pancreas, bone, adrenal glands, and genitourinary tract. The most common site of involvement outside the genitourinary tract is the gastrointestinal tract. We report a case of colonic malakoplakia who presented with unintentional weight loss, abdominal pain, and chronic diarrhea.

Cerebral malakoplakia is a very rare chronic inflammatory disease. We herein report the case of a 49-year-old female who presented with a slowly progressive speech disturbance and right hemiparesis. Computed tomography and magnetic resonance imaging showed irregular enhanced mass lesions with numerous scattered areas of calcification in the left insula, thalamus and basal ganglia.
