The mechanism of the leukochemotactic activity of staphylococcal protein A (pA) has been studied by in vitro and in vivo experiments. Protein A alone or mixed with heat-inactivated serum induced no migration of polymorphonuclear leukocytes, demonstrating that pA is not a cytotaxin, but a cytotaxigen. Protein A, activating both pathways of the C system, induced chemotaxis in a C4 deficient serum, but not in a C5 deficient serum. This shows that the chemotactic mediator elaborated is a split product of C5, i.e. C5a. The chemotactic activity of pA observed in the presence of normal sera (in vitro) and in wound chambers (in vivo) was exclusively caused by C activation via reaction with Fc.

Coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 and long COVID can present with nonspecific symptoms resembling adrenal insufficiency. This similarity of symptoms means that adrenal insufficiency hidden among nonspecific manifestations of COVID-19 may pass underrecognized. We present the case of a 53-year-old Japanese man who developed isolated adrenocorticotrophic hormone (ACTH) deficiency (IAD) and acute adrenal insufficiency after COVID-19, thus mimicking prolonged symptoms of COVID-19.

Alpha-1-Antitrypsin (A1AT) deficiency is a common hereditary disorder associated with increased risk of developing chronic obstructive pulmonary disease (COPD). Many individuals with severe A1AT deficiency go undiagnosed, or are diagnosed late, and fail to benefit from disease-specific counseling and modifying care. Since the 2012 Canadian Thoracic Society (CTS) A1AT deficiency clinical practice guideline, new approaches to optimal diagnosis using modern genetic testing and studies of A1AT augmentation therapy have been published.
