Pelvic fracture in multiple trauma: are we still up-to-date with massive fluid resuscitation?

Injury

October 2014

Department of Trauma, Hand and Reconstructive Surgery, University of Saarland, Kirrbergerstraße 100, 66421 Homburg/Saar, Germany. Electronic address:

Until today the mortality of complex pelvic trauma remains unacceptably high. On the one hand this could be attributed to a biological limit of the survivable trauma load, on the other hand side an ongoing inadequate treatment might be conceivable too. For the management of multiple trauma patients with life-threatening pelvic fractures, there is ongoing international debate on the adequate therapeutic strategy, e.

Aim: Administration of 100% oxygen [hyperoxic ventilation (HV)] has been proven to ameliorate oxygen transport, tissue oxygenation and survival in different models of extreme normovolemic and hypovolemic anaemia. However, up to date, it is unknown whether HV is also able to improve outcome of extreme anaemia if myocardial oxygen consumption is contemporaneously increased by tachycardia. Therefore, we investigated the influence of HV on the 6-h survival rate during extreme anaemia and aggravated by experimentally induced tachycardia in a prospective, randomized study in a pig model of critical anaemia.

Management of anemia in heart failure.

Curr Opin Cardiol

March 2010

Department of Medicine, Section of Cardiology, University of Illinois at Chicago, Chicago, Illinois, USA.

Purpose Of Review: Anemia is a relatively common finding in heart failure. Anemia in heart failure patients has been independently associated with reduced exercise tolerance, increased heart failure hospitalizations and increased all-cause mortality. Anemia would appear to be a reasonable treatment target for patients with heart failure.

[An update on the treatment of central retinal vein thrombosis (retinal vein occlusion)].

Oftalmologia

November 2000

Clinica de Oftalmologie, Spitalul Municipal Constanţa.

The paper reviews the actual therapeutical means in a severe invalid eye disease. The treatment is not able to improve the retinal blood flow, being more effective on the main symptoms and complications. The isovolumetric hemodilution method as an up-to-date method in the treatment of central retinal vein occlusion, appears to bring better recuperation hopes, by a higher improvement of the local anatomical and hemodynamical factors.

[Hemodilution therapy with neuron metabolism specific therapy in ischemic stroke--encouraging results of a comparative study].

Wien Med Wochenschr

November 1996

Aus der Neurologischen Abteilung, Ludwig-Boltzmann-Institut für Neuro-Rehabilitation und -Prophylaxe.

Treating ischemic stroke we could compare a (control-)group of 318 patients under up to date hemodilution therapy to a group of 100 patients receiving exactly the same treating scheme but including an add-on administering of Cerebrolysin. It proved a statistically validated better outcome within the Cerebrolysin add-on treated group evaluated by the Barolin-Neuro-Rehabilitation Scale. 1) More effect on social and occupational parameters than on motor functions, but also including some of those.
