Severe edema-proteinuria-hypertension (EPH) gestosis (pre-eclampsia) appears in at least two forms (A and B). The A type is mostly complicated by fits, while the B type is characterized by multiple organ failure. The perinatal mortality rate is especially high in the B group. All gravid women with severe EPH gestosis benefit from blood volume expansion therapy.

The paper is aimed at evaluation of certain indices of cellular immunity in pregnant women with gestosis superimposing pre-existing renal diseases and comparing them with the immunological condition of women with essential gestosis. T lymphocyte subpopulations and lymphocyte proliferative responses to mitogens (phytohemagglutinin--PHA, concanavalin A--Con A and pokeweed mitogen--PWM) in the fetal calf serum and autological sera were studied. The groups of examined pregnant women in the 3rd trimester did not vary with regard to gestation age, calendar age and to the severity of edema (E), proteinuria (P) and hypertension (H) gestosis symptoms.

With enzyme linked immunosorbent assays, using a solid phase Clq method, we determined free circulating immunocomplexes (CIC) in the serum of 47 pregnant women. In 30 normal pregnant women without EPH gestosis no CIC could be found. 3 normal pregnant women showed temporary CIC after viral infections.

Five cases of pregnant woman with symptoms of right upper quadrant pain, hemoconcentration, liver dysfunction, and thrombocytopenia are presented as representative of impending gestosis. Plasma volume expansion achieved by either bed rest or intravenous albumin administration appeared to be effective therapy. It is presumed that impending gestosis represents an early form of severe toxemia (edema/proteinuria/hypertension [EPH[ gestosis).
