Adrenal haemorrhage is not an exceptional event in the newborn. The probable predisposing factors are difficult delivery, fetal hypoxia, trombocytopenia and coagulation defects. The recommended therapeutical approach is conservative, even if some Authors prefer surgical evacuation of the haematoma. This report was prompted by our recent experience of a case of unilateral adrenal haemorrhage in a newborn. The methods useful in the diagnosis are thoroughly evaluated and discussed with special emphasis on echography which holds an important position in the early diagnosis of this condition.

Background: Liver injury manifesting as hepatic enzyme abnormalities, has been occasionally identified to be a feature of primary or secondary Addison's disease, an uncommon endocrine disease characterized by adrenal insufficiency. Insulinoma-associated protein 1 (INSM1) is a relatively new immunostain used in the diagnostic assessment of tumors with neuroendocrine differentiation. While INSM1 positivity has been described in some non-neuroendocrine neoplasms, reactivity in red blood cells (RBCs) has only been anecdotally noted in one prior study without description of the degree/extent of staining. INSM1 staining in nucleated erythroid precursors has not been previously reported.
