Objectives: To analyse the effects on respiratory function, lung volume and the regional distribution of ventilation and perfusion of routine postural repositioning in mechanically ventilated critically ill patients.
Methods: Prospective descriptive physiological study. We evaluated gas-exchange, lung mechanics, and Electrical Impedance Tomography (EIT) determined end-expiratory lung impedance and regional ventilation and perfusion distribution in five body positions: supine-baseline (S1); first lateralisation at 30° (L1); second supine position (S2), second contralateral lateralisation (L2) and third final supine position (S3).

The oxygen reserve index (ORi) is a novel, non-invasive parameter that estimates arterial oxygen partial pressure (PaO) during hyperoxia when the fraction of inspired oxygen (FiO) is elevated. This study aimed to assess the utility of the ORi/FiO ratio as an index for quantifying F-shunt, serving as an estimate of venous admixture. Anesthetic records were reviewed from 44 dogs undergoing general anesthesia and requiring arterial catheterization.

Background: While patients are assessed for their ability to tolerate surgery through physiologic evaluations such as pulmonary function tests, ventilation-perfusion scans, and exercising testing, some patients still require home oxygen therapy after pulmonary resection. It is not well understood what the associated risk factors are, how long patients need supplemental oxygen, and if this requirement is associated with worse long-term outcomes. Given these knowledge gaps, we sought to conduct a systematic review of pulmonary resections and new postoperative home oxygen requirement.

Introduction: Preclinically, 24-hour continuous Ex-Situ Lung Perfusion (ESLP) is the longest duration achieved in large animal models and rejected human lungs. Here, we present our 36-hour Negative Pressure Ventilation (NPV)-ESLP protocol applied to porcine and rejected human lungs.
Methods: Five sets of donor domestic pig lungs (45-55 kg) underwent 36-hour NPV-ESLP.

Objectives: This study investigates the impact of bilateral antegrade cerebral perfusion (ACP) time on outcomes in aortic arch surgery.
Methods: In total, 961 patients underwent either hemiarch (n = 385) or total arch replacement (n = 576) with bilateral ACP and moderate hypothermia management between 2006 and 2020 across 2 aortic centres. ACP time was categorized into 4 groups (≤30 min: n = 169, 30-60 min: n = 298, 60-90 min: n = 261, >90 min: n = 233).
