In nine patients with syndrome of ventricular over-excitation of intracavitary electrocardiographic investigation was carried out, including recording of Hiss bundle potentials, right and left auricle, programmed, increment and high frequent stimulations. The anatomic characteristic of the additional connection, on the base of the deformation of QRS-complex and the type of the retrograde conduction in case of ventricular stimulation showed that the connection started from the right auricle. In five--it terminates in the left ventricle, and in four--in the right. In the majority of the patients it is with an effective refractory phase under 300 msec., corresponding to the electrophysiological characteristic of a direct connection between auricles and ventricles. The problem of the origination of the phenomenon of circling excitation in the patients with the syndrome of ventricular overexcitation is discussed.

Chronic heart failure (CHF) is a complex clinical syndrome resulting from various cardiac diseases, characterized by weakened cardiac pumping capacity and inadequate blood supply to body tissues. This study aims to investigate the expression and clinical implications of pro-B-type natriuretic peptide (pro-BNP) and soluble suppression of tumorigenicity 2 (sST2) in CHF to explore their potential in early diagnosis and severity assessment of the pathological condition. This study included 146 CHF patients treated at our hospital from January 2022 to December 2023, who were classified in the observation group, and 150 concurrent healthy people categorized in the control group.

Unlabelled: A 93-year-old man was admitted to our hospital with complaints of pain and coldness in both lower legs. He was diagnosed with arterial occlusive disease of the lower extremities. There were no obvious stenosis or occlusion of lower extremity arteries, which would be indications for surgery, and drug therapy was started.

Unlabelled: Heparin-induced thrombocytopenia (HIT) is an immune-mediated disease with severe thromboembolic complications. HIT during percutaneous coronary intervention (PCI) can be fatal without prompt treatment. We report an unusual case of HIT observed during PCI for acute coronary syndrome (ACS).

Absent pulmonary valve syndrome, commonly linked with tetralogy of Fallot and ventricular septal defect, is a rare congenital condition. It is exceedingly rare to have an isolated absent pulmonary valve with an intact ventricular septum without cardiovascular shunt lesions, such as an atrial/ventricular septal defect or patent ductus arteriosus. This report presents a case of such rarity involving a young child with recurrent lower respiratory tract infections.

Miyazaki syndrome is a rare complication in patients with ventricular shunts, characterised by cervical myelopathy from cerebrospinal fluid overdrainage. Here we report a case of a young woman with a history of hydrocephalus who had a ventricular shunt placed in infancy, presenting to our department with signs and symptoms of myelopathy due to overshunting. This diagnosis is challenging because of the frequent absence of typical signs of intracranial hypotension, leading to delays in treatment, which carries the risk of potentially devastating consequences.
