Long neglected, the significance of mitral annulus calcification had now been re-evaluated with the help of echocardiography. An unusual case with clinical symptoms mimicking rheumatic mitral valve disease has enabled the authors to discuss the acoustic manifestations of this entity, its evolutive potential and its causative factors. It is concluded that mitral annulus calcification should no longer be considered as a benign feature of the ageing heart.

Undiagnosed cases have presumably increased in the same proportions, and rare complications are at particularly high risk of being unrecognised. Objectives: The ValveClamp system (Hanyu Medical Technology) is a novel transcatheter edge-to-edge repair (TEER) system designed for ease of operation; however, there is a lack of data on its application in secondary mitral regurgitation (SMR). The authors report the mid-term outcomes of TEER using the ValveClamp system in SMR.
Methods: The study prospectively analyzed consecutive severe SMR patients who underwent transapical ValveClamp implantation at 10 Chinese centers.

We present 2 cases of caseous mitral annulus calcification (MAC) in which one patient was asymptomatic whereas the second experienced left hemianopsia. Both patients underwent transthoracic and transesophageal echocardiography exams which revealed severe MAC with a mass consistent with caseous MAC. A mobile component of the caseous MAC was observed in the patient with left hemianopsia.
