In the following summary, we give the definition and interpretation of sclerotherapeutic failure, and the procedure to be observed in such cases. Failure will appear either as postsclerotic relapse, phlebitis at the site of injection, or absence of sclerosis after adequate dosing of sclerosants. In such cases, there are three possibilities: 1. the patient is less sensitive to the sclerosant; 2. the patient has developed sclero-resistance; 3. the patient has an initially concealed saphenous insufficiency. We give you the procedure to follow in each of the three cases.

Background: To explore the short-term efficacy and safety of endoscopic low dose injection of sclerotherapy and cyanoacrylate for gastric varices (GVs) combined with endoscopic variceal ligation (EVL) for esophageal varices (EVs) in cirrhosis with type GOV1 varices.

Methods: A total of 521 patients with cirrhosis and GOV1 varices, who were divided into emergence endoscopy treatment layer and secondary prophylaxis for rebleeding layer, were selected. All patients underwent combined therapy or EVL alone (ligation therapy) for the treatment of type GOV1 varices.

Pearls and Pitfalls in the Management of Vascular Anomalies.

Semin Intervent Radiol

August 2024

The AVM Center of New York at Lenox Hill, Lenox Hill Hospital, Northwell Health, New York, New York.

Recent advancements in endovascular techniques have transformed the management of vascular malformations, shifting away from traditional surgical resections to less invasive methods like embolization and sclerotherapy. Vascular malformations are congenital anomalies resulting from embryonic angiogenesis failures, often misdiagnosed but manageable through a multidisciplinary approach. These malformations range from high-flow arteriovenous connections to low-flow venous, lymphatic, or mixed lesions, each requiring specific diagnostic and treatment strategies.

Article Synopsis
  • Spontaneous hemorrhage from varicose veins can be a dangerous complication, and a two-step clinical protocol was developed to prevent immediate bleeding and ensure long-term results.* -
  • The protocol involved a prospective registry of patients, where they received ultrasound assessments and immediate treatment through foam sclerotherapy, followed by planned endovenous ablation within eight weeks.* -
  • Out of 59 patients treated, 95% had identifiable underlying issues, and while 52 received the immediate sclerotherapy, only 7 did not; results indicated no recurrent bleeding post-treatment, with a 7% rate of late hemorrhage in long-term follow-up.*

Factors affecting the technical outcome of catheter-directed sclerotherapy for ovarian endometriomas.

Eur J Radiol

December 2024

Department of Radiology, Severance Hospital, Research Institute of Radiological Science, Yonsei University College of Medicine, Seoul 03722, Korea. Electronic address:

Article Synopsis
  • The study aimed to identify factors that affect the success of catheter-directed sclerotherapy (CDS) for treating ovarian endometriomas and to propose criteria for patient selection.
  • It analyzed data from 323 women who underwent CDS from 2015 to 2023, focusing on endometrioma characteristics and procedural techniques.
  • Key findings included that endometriomas smaller than 3 cm, those with T2 dark signal intensity, and using transvaginal access were linked to higher chances of technical failure during the procedure.

Article Synopsis
  • The study aimed to analyze pregnancy and neonatal outcomes for fetal cystic lymphatic malformations (LMs) not located in the posterior neck to help in patient counseling.
  • A systematic review of 65 studies involving 96 fetuses was conducted, revealing a 94% live birth rate and a typical diagnosis around 25.5 weeks, mainly in the anterior neck.
  • Findings indicated that LMs generally increase in size before delivery, have few complications, and most surviving infants show normal development, with only a small percentage experiencing significant health issues post-birth.*
