Oesophageal duplication cysts are a subtype of foregut cysts, associated with the presence of ectopic gastric mucosa. Gastric acid secreted by this mucosa can lead to complications. We report one such unusual case of complication leading to the formation of broncho-oesophageal fistula in a duplication cyst.

Anomalies of the fourth branchial cleft are exceedingly uncommon, presenting with a diverse array of clinical manifestations. The majority of branchial cleft anomalies, approximately 95%, are of the second type, with a mere 2% attributed to the fourth type. The latter is notably more prevalent on the left side, with reports indicating an 85% incidence.

We present a case of a three-month-old male who presented with a cervical esophageal duplication cyst requiring early surgical intervention. The patient presented with feeding difficulties, poor weight gain, and respiratory distress. Due to the position of the cervical esophageal duplication cyst and airway compression, this unique case required a multidisciplinary surgical approach involving both otolaryngology and cardiothoracic surgery.
