An examination of 200 patients with cerebral arachnoiditis has shown that the meningeal adhesions and cysts revealed on pneumoencephalography may be consequences of current or terminated chronic leptomeningitis and chorioependimitis. Characteristic of current inflammatory process are variability and progress of the neurological symptoms, increase of cytosis and, sometimes, of protein content in the CSF, as well as some hematological signs of an inflammation, such as relative lymphocytosis, positive test for the C-reactive protein, and dysproteinemia. Such a differentiation of cerebral arachnoiditis has enabled the authors to choose more rational therapeutic measures and to give an adequate evaluation of their efficacy.

Cerebral venous thrombosis (CVT) is a rare and potentially critical cerebrovascular disease involving intracranial dural sinuses and veins. The diagnosis is a stepwise pathway starting from clinical suspicion and employing several neuroradiological techniques, mainly Computed Tomography (CT)-based and Magnetic Resonance Imaging (MRI)-based modalities. The neuroradiological findings, both in the diagnostic phase and in the follow-up phase, may provide some results at risk for misdiagnosis.

Shunt dependence syndrome is a serious long-term complication characterized by symptoms and signs of increased intracranial pressure with normal-sized lateral ventricles after several years of arachnoid cyst-peritoneal shunting. It is easy to misdiagnose and overlook when combined with sinus stenosis, thus delaying treatment. Calcified chronic subdural hematoma (CCSDH) is a rare condition characterized by the accumulation of calcified blood between the dura mater and arachnoid membrane, typically following remote trauma. These lesions often present as space-occupying, extra-axial masses over the cerebral convexity and can mimic extra-axial tumors, such as calcified meningiomas. A 73-year-old male with a history of prostate cancer, hypertension, and hyperlipidemia presented with vision changes and mild papilledema.
