Purpose: To describe a rare case of infectious keratitis secondary to Brevundimonas diminuta, a gram-negative bacillus with fluoroquinolone resistance and rare clinical isolation.
Methods: A 50-year-old man with contact lens overuse presented with a large corneal ulcer and hand motion visual acuity. Initial treatment with fortified topical tobramycin and vancomycin yielded slow improvement, and initial culture grew Staphylococcus epidermidis, Staphylococcus hominis, and Corynebacterium bovis.
"Biliary-cast syndrome" ("BCS") is most often encountered in clinical practice as a complication after liver transplantation, there are also described cases of biliary-cast syndrome in patients who did not undergo liver transplantation, isolated cases of "BCS" developing in patients with acute pancreatitis, choledocholithiasis are described in literature. Ischemic damage to bile duct epithelium with development of cholestasis and retrograde biliary tract infection are considered as the main etiological factors. This work presents a clinical case of "Biliary-cast syndrome" in a patient with acute biliary pancreatitis and pulmonary embolism.

Background: The superior vena cava (SVC) acts as a non-pulmonary vein (PV) trigger for atrial fibrillation (AF) in 2%-6% of patients and harbours 25%-40% of non-PV foci. Approximately 10% of patients with AF have epicardial connections (ECs) between the atrium and PV inside the PV isolation lines, which are associated with AF recurrence. However, the contribution of EC(s) between the SVC and right atrium (RA) to subsequent AF remains unknown.
