The report concerns a man with onset of diabetes in 1921 and treated with insulin since 1922. In 1981, at the age of 75, he is in excellent health, with no signs of significant diabetic organ injuries. Among other factors, this benign course may be due to retention, until now, of some endogenous insulin production.

Changes in CD4+CD25HIGH T cells and TGFβ1 levels in different stages of adult-onset type 1 diabetes.

University of Belgrade, Faculty of Medicine, University Clinical Centre of Serbia, Clinic for Endocrinology, Diabetes and Metabolic Diseases, Belgrade.

Background: Previous studies suggested an important role of impairments in T cell subsets in different stages during type 1 diabetes (T1D) development, while data regarding CD25high T cells and transforming growth factor b1 (TGFβ1), both T regulatory associated, remains controversial. We analyzed the level of (a) CD25high T cells (b) TGFβ1 in 17 first-degree relatives of patients with T1D in stage 1 (FDRs1) (GADA+, IA-2+); 34 FDRs in stage 0 (FDRs0) (GADA, IA-2); 24 recent-onset T1D in insulin-requiring state (IRS); 10 patients in clinical remission (CR); 18 healthy, unrelated controls (CTR).

Methods: T cell subsets were characterized by two-color immunofluorescence staining and flow cytometry; TGFβ1 was determined by ELISA, GADA, and IA-2 by RIA.

Primary care clinicians (PCCs) manage 90% of patients with diabetes, 30% of whom require insulin with a substantial number poorly controlled because of the challenges that PCCs face (time constraints and lack of experience). The author has developed Federal Drug Administration cleared and Conformite Europeenne mark registered comprehensive computerized insulin dose adjustment algorithms (CIDAAs) to enable PCCs to significantly lower HbA1c levels in insulin-requiring patients. Reports sent to PCCs contain scatter plots of glucose readings, their organization into pre- and postprandial and before bedtime values, their analyses, and recommendations for insulin dose adjustments (if indicated) that the PCC can accept or modify.

We report the case of a patient with type 2 diabetes mellitus (T2DM) on insulin therapy with a history of recurrent and severe hypoglycemia related to lipodystrophy with an uncommon clinical presentation. This was the case of a 67-year-old female with type 2 diabetes hospitalized for the exploration and management of severe and recurrent hypoglycemia. Her diabetes has been evolving since the age of 40 years and was complicated by minimal retinopathy.

Aims: Continuous glucose measurement (CGM) systems are increasingly utilised by people with diabetes mellitus (DM) and less is known about usage behaviour. Therefore, this study aims to analyse additionally utilisation of blood glucose measurement (BGM) for insurants who are using CGM.

Methods: The study used secondary data, health claims data from the AOK Saxony-Anhalt (Germany), from 2016 to 2021, analysing a sample of 52,296 individuals with insulin-requiring DM.

Article Synopsis
  • Metabolic and bariatric surgery (MBS) can lead to postoperative complications, particularly acute kidney injury (AKI), with historical rates ranging from 5.8% to 8.6% but more recent findings show a lower incidence of 3% among participants.
  • The study aimed to identify the risk factors contributing to AKI after MBS, which included male gender, hypertension, and extended surgical duration, using data from adult patients treated between 2008 and 2022.
  • Notably, among those who experienced AKI, 13.7% required dialysis, and 5.9% progressed to chronic renal failure necessitating a transplant, highlighting the critical nature of monitoring kidney health post-surgery.
