Tc-99m-sulfur colloid reticuloendothelial images and percutaneous liver biopsies are correlated in 54 patients who presented with signs and symptoms of subacute hepatic necrosis or chronic active hepatitis. The scored Tc-99m-sulfur colloid scan is compared to the histological severity of disease. Serial scans and biopsies are compared. In patients with significant hepatocellular disease, the liver-spleen scan did not correlate well with the histological severity of disease. Changes in the spleen-to-liver ratio are more common than those in the liver or increased bone marrow activity, and appear earlier. In serial studies, changes in the Tc-99m-sulfur colloid scans correlate weakly with changes in histology.

Lymphoscintigraphy With SPECT-CT in Detecting the Site of Chyle Leak in Postoperative Patient.

Clin Nucl Med

December 2024

Department of Surgical Oncology, M.N.B.C.I, Sadhu Vaswani Mission's Medical Complex, Pune, India.

Here is a case of chyle leak post McKeown esophagectomy. Lymphoscintigraphy with 99m Tc-filtered sulfur colloid revealed tracer accumulation along the thoracic duct and in the left hemithorax. Precise localization of leak was done by SPECT-CT imaging.

Spectrum of Heterotopic and Ectopic Splenic Conditions.

Radiographics

November 2024

From the Department of Radiology, University of Wisconsin School of Medicine and Public Health, E3/311 Clinical Science Center, 600 Highland Ave, Madison, WI 53792-3252 (L.W.N., M.G.L., P.J.P.); Mallinckrodt Institute of Radiology, Washington University School of Medicine, St Louis, Mo (S.M.B., S.B.); and American College of Radiology (ACR) Institute for Radiologic Pathology (AIRP), Silver Spring, Md (M.G.L., P.J.P.).

A spectrum of heterotopic and ectopic splenic conditions may be encountered in clinical practice as incidental asymptomatic detection or symptomatic diagnosis. The radiologist needs to be aware of these conditions and their imaging characteristics to provide a prompt correct diagnosis and avoid misdiagnosis as neoplasm or lymphadenopathy. Having a strong knowledge base of the embryologic development of the spleen improves understanding of the pathophysiologic basis of these conditions.

Characterizing Normal Upper Extremity Lymphatic Flow with Tc In-House Dextran: A Retrospective Study.

Diagnostics (Basel)

September 2024

Division of Nuclear Medicine, Department of Diagnostic and Therapeutic Radiology, Faculty of Medicine Ramathibodi Hospital, Mahidol University, Bangkok 10400, Thailand.

Lymphoscintigraphy evaluates the lymphatic system using radiocolloid compounds like Tc-sulfur colloid and Tc-nanocolloid, which vary in particle size and distribution timing. A local in-house Dextran kit (15-40 nm) was developed in 2005 and began clinical use in 2008 to localize sentinel lymph nodes; diagnose lymphedema; and detect lymphatic leakage. The normal drainage pattern remains unexplored.

Background: Lymphatic mapping with SPECT-CT has been demonstrated to accurately define lymphatic drainage patterns in oropharyngeal cancer but there has yet to be a study demonstrating its feasibility across multiple institutions.

Methods: Twelve adult patients with lateralized oropharyngeal carcinoma (T1-T3) who were planned for definitive or adjuvant radiotherapy without contralateral nodal disease underwent injection of 99-m technetium sulfur colloid followed by static planar lymphoscintigraphy to verify tracer migration, and SPECT-CT acquired at 30 ± 15 min (optional) and 3 h (±1 h) (mandatory time-point).

Results: All 12 patients completed the study with 7/12 patients having the injections performed under local anesthetic and 5 patients requiring general anesthetic.
