The effects of long-term treatment with azathioprine and prednisolone on T- and B-lymphocytes were studied in 52 patients with lupus nephritis (LN) and chronic glomerulonephritis (GN). The effect of azathioprine on lymphocyte populations was dose dependent; high doses decreased the number of T- and particularly, B-cells, while smaller doses produced a selective depletion of B-cells. The changes in T- and B-cells during prednisolone treatment were variable with alternating increases and decreases in their numbers. The patients with increased B-lymphocyte levels showed the best response to immunosuppressive therapy.

The APLAR has published a set of recommendations on the management of systemic lupus erythematosus (SLE) in 2021. The current consensus paper supplements and updates specifically the treatment of lupus nephritis (LN) according to two rounds of Delphi exercise from members of the APLAR SLE special interest group, invited nephrologists, histopathologists, and lupus nephritis patients. For initial treatment of LN, we recommend a combination of glucocorticoids (GCs) with cyclophosphamide (CYC), mycophenolate mofetil (MMF), or the calcineurin inhibitors (CNIs) as first-line options.

CVID (common variable immunodeficiency) is associated with a variety of gastrointestinal disorders including those mimicking Crohn's disease and ulcerative colitis. At present there is no clear trial data for the treatment of CVID enteropathy. There are no specific recommendations for treatment; however, it is commonly treated in a similar manner to inflammatory bowel disease, with corticosteroids, 5-aminosalicylates (5-ASA), azathioprine and anti-TNF therapy all being used.
