Osteoporosis in a painful hip is usually a sign of inflammation or tumor. There exists, however, a self-limited clinical entity that presents with pain in the hip and where the only roentgenological manifestation is osteoporosis. Eight patients with this disease are described: five men and three women, ranging in age from 29 to 50 years. In four patients, the onset of disease was related to sprain, pregnancy, or delivery. Laboratory analyses and the clinical course of the disease excluded inflammation or a neoplasm as the cause. All patients fully recovered with 12 month, with no sequelae. The etiology of the disease remains unknown.

Idiopathic transient osteoporosis of the hip (ITOH) is a rare and self-limiting condition of unknown origin, typically responding well to conservative treatment. It is characterized by progressive pain, claudication, and osteoporosis of the femoral head, while the joint line remains preserved. Early clinical and radiological findings can be misinterpreted as aseptic necrosis, infection, or neoplastic processes, making careful exclusion of these conditions essential.

Angelica gigas Nakai (AGN) root is a medicinal herbal widely used in traditional medicine in Korea. AGN root ethanolic extracts have been marketed as dietary supplements in the United States for memory health and pain management. We have recently reviewed the pharmacokinetics (PK) and first-pass hepatic metabolism of ingested AGN supplements in humans for the signature pyranocoumarins decursin (D, C 1x), decursinol angelate (DA, C ~ 10x) and their common botanical precursor and hepatic metabolite decursinol (DOH, C ~ 1000x).

We present a case of a 73-year-old woman with a medical history significant for hyperlipidemia, on pravastatin, who developed Takotsubo cardiomyopathy following a diagnosis of osteoporosis. She presented to the Emergency Department with acute transient left arm pain that resolved spontaneously. Investigations revealed elevated troponin levels, non-specific electrocardiographic changes, no significant coronary artery disease on angiography, and left ventricular systolic dysfunction, findings consistent with Takotsubo cardiomyopathy.
