The relationship between maxillofacial trauma and alcohol abuse has been well documented. The onset of acute alcohol withdrawal following the injury can seriously complicate and compromise the treatment of the maxillofacial injuries as well as be potentially lethal for the patient. A clear understanding of the biochemistry, development, signs and symptoms, and management of acute alcohol withdrawal is necessary for the oral and maxillofacial surgeon.

Ethylene glycol (C₂H₆O₂), a toxic alcohol commonly found in automotive antifreeze, de-icing solutions, and industrial coolants, can cause severe toxicity when ingested. Due to its sweet taste, it is often consumed accidentally or intentionally, leading to life-threatening consequences such as metabolic acidosis, acute kidney injury (AKI), and mortality. Prompt diagnosis and early treatment with antidotes such as fomepizole or ethanol, combined with hemodialysis, are essential in preventing severe outcomes.

: A 65-year-old male veteran presented to the Veterans Affairs Boston Healthcare System (VABHS) emergency department with progressive fatigue, dyspnea on exertion, lightheadedness, and falls over the last month. New bilateral lower extremity numbness up to his knees developed in the week prior to admission and prompted him to seek care. Additional history included 2 episodes of transient loss of consciousness resulting in falls and a week of diarrhea, which had resolved.
