A large pedunculated myxoid liposarcoma of the cervical esophagus is presented. Because of its long pedicle, the presenting symptoms were primarily respiratory rather than digestive. The tumor lent itself to local excision rather than esophageal resection, given the absence of involvement of its pedicle and the resultant wide margin of excision. Review of the literature did not disclose a similar case.

This case report presents the successful endoscopic submucosal dissection (ESD) of a well-differentiated esophageal liposarcoma in a 51-year-old male with persistent dysphagia. The cause was initially diagnosed as a 10 cm pedunculated lesion extending from the upper esophageal sphincter to the mid-esophagus. An ESD was chosen over traditional surgery because it is less invasive.

Liposarcomas are a subtype of soft tissue sarcomas arising from lipoblasts, a mesenchymal cell lineage that commonly arises from deep tissues of the body. Though these are the most common subtypes, their early diagnosis still remains a challenge due to their varied presentation as a soft benign appearing growth. The pleomorphic variant has complex management due to its high recurrence rate and resistance to chemoradiation.

Background: Liposarcoma is one of the most common soft tissue sarcomas, but is extremely rarely found in the esophagus. There have been no reports of esophageal liposarcoma together with superficial carcinoma of the esophagus.
