IgG and IgM anti-PP1Pk antibodies of human sera from p individuals were examined for their ability to agglutinate red blood cells (RBC) of various P phenotypes and to mediate ADCC of these RBC by human effector cells. Agglutinating antibodies were found in either the IgG or the IgM fraction, while ADCC active antibodies were found mainly in the IgG fraction and were essentially cytotoxic with Pk RBC. The possible relationship of these antibodies with early abortions of p mothers is discussed.

We report the case of a 5-year-old girl with a rare P-null phenotype who presented for a neurosurgical procedure at our center. The case is unique as this patient was one of the two P-null phenotype cases reported in India and we report how we could successfully arrange a rare blood unit for her. As it was challenging to find a donor for her, autologous blood was collected.

Anti-PP1P alloimmunization is rare given ubiquitous P1PK expression. Prevention of recurrent miscarriages and hemolytic disease of the fetus and newborn (HDFN) in pregnant individuals with anti-PP1P antibodies has relied upon individual reports. Here, we demonstrate the successful management of maternal anti-PP1P alloimmunization in a 23-year-old, G2P0010, with therapeutic plasma exchange (TPE), intravenous immunoglobulin (IVIG), and monitoring of anti-PP1P titers.

Pregnant women with p phenotype, who lack antigens P, P1, and Pk, spontaneously form anti-PP1Pk antibodies whose primary target is the placenta. The risk of miscarriage in these women is 50%-70% and reaches 87% in the second trimester. The therapies aim to reduce the titer of antibodies early in pregnancy.

The histo-blood group antigens P, P1 and Pk are a closely related set of glycosphingolipid structures expressed by red blood cells and other tissues. None of these three characters is expressed on p cells, a null phenotype that arises in the context of homozygous mutation of the A4GALT gene. Subjects with p phenotype spontaneously develop a natural alloantibody named anti-PP1Pk, which is a mixture of IgG and IgM against P1, P and Pk.

Objective: To analyze the serological characteristics and molecular mechanism for an individual with p phenotype.
Methods: An individual with p phenotype upon blood group identification at Jiaxing Blood Center in May 2021 was analyzed. ABO, RhD and P1PK blood groups and irregular antibodies in her serum were identified using conventional serological methods.
