A case of localized cranial Wegener granulomatosis was imaged with high resolution computed tomography. The mass extended from the infratemporal fossa, through the basal foramina, and into the cavernous sinus, causing complete occlusion of the internal carotid artery. Computed tomography demonstrated absence of bone destruction and a normal parotid gland and nasopharynx. Although uncommon, Wegener granulomatosis should be considered in the differential diagnosis of a parapharyngeal mass with intracranial extension.

Anti-neutrophil cytoplasmic antibodies (ANCA)-associated vasculitides are a group of rare diseases that cause inflammation of small to medium vessels. They comprise granulomatosis with polyangiitis (GPA), microscopic polyangiitis (MPA), and eosinophilic granulomatosis with polyangiitis (EGPA). As the signs and symptoms are varied, the diagnosis of vasculitis is complex and challenging; however, there are type-specific clinical presentations that can be factored into the existing classification criteria.

A 78-years-old man was treated for asthma and pansinusitis for >5 years, and mepolizumab was initiated two years previously. Background: Eosinophilic granulomatosis with polyangiitis (EGPA) is an extremely rare type of vasculitis characterized by inflammation within small blood vessels or tissues that may cause damage to the lungs, heart, kidneys, and other organs. Here, we present a rare case of EGPA with cardiac involvement that presented with acute heart failure.
Clinical Findings: A 44-year-old woman with a history of bronchial asthma and sinusitis presented with fever, shortness of breath, fatigue, unintentional weight loss, and polyarthritis.
