[Anterior epidurography with verographin].

Zh Vopr Neirokhir Im N N Burdenko

Published: May 1983

Direct contrasting of the anterior epidural space with 9-10% verografin solution was conducted in 123 patients (120 with discogenic radiculitis, 2 with tumors, and 1 with injury to the spine). The epidural space was contrasted for a distance of 3-4 intervertebral spaces and no stimulation of the epidural structures occurred. The epidurograms demonstrated convincing information concerning the character of the protrusion and prolapse of the intervertebral discs and the existence of block of the epidural space in tumors. Phenomena of "spinal epilepsy" occurred in 5 cases, which regressed rapidly after a complex of measures was applied. Anterior epidurography with water-soluble contrast media of low concentration is an informative and safe method.

Intradural lumbar disc herniation (ILDH) is a very rare condition, with cerebrospinal fluid (CSF) leakage as a postoperative complication. The central canal of the conus medullaris was reported to communicate with the subarachnoid space through a caudal aperture; however, this aperture has never been observed in vivo. Herein, we report a case of L1/2 ILDH with postoperative spinal adhesive arachnoiditis and syringomyelia in which the communication considered to be a caudal aperture was detected.

Azygos Vein Stenosis in Frontotemporal Dementia Sagging Brain Syndrome.

AJNR Am J Neuroradiol

January 2025

Ataxia Center, Cognitive Behavioral Neurology Unit, Laboratory for Neuroanatomy and Cerebellar Neurobiology, Department of Neurology (J.D.S., Massachusetts General Hospital and Harvard Medical School, Boston, Massachusetts.

Background And Purpose: Symptoms indistinguishable from behavioral-variant frontotemporal dementia (bvFTD) can develop in patients with spontaneous intracranial hypotension associated with severe brain sagging. An underlying spinal CSF leak can be identified in only a minority of these patients and the success rate of nondirected treatments, such as epidural blood patching and dural reduction surgery, is low. The disability associated with bvFTD sagging brain syndrome is high and, because of the importance of the venous system in the pathophysiology of CSF leaks in general, we have investigated the systemic venous circulation in those patients with recalcitrant symptoms.

Background Context: On radiopathological examination of spinal tuberculosis (TB), two predominant forms are known: dry and wet types. Wet TB, as the name suggests, has abscess formation as its predominant presenting feature and is the exudative form; dry TB includes caseation and sequestration with minimal exudate. Dry TB often exhibits poorer recovery patterns than the wet counterparts, which can be possibly ascribed to vasculitis, ischemia, or tubercular myelitis, rather than isolated mechanical compression.

This study aims to thoroughly investigate the clinical presentation, duration of symptoms, radiological aspects of posterior epidural migration of disc fragments (PEMDF), and assess various treatment options and their impacts on patient functionality. A systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. We conducted a comprehensive search in PubMed, Web of Science, and Scopus from inception to March 2024.

Background: This study aimed to assess the feasibility of real-time ultrasound-guided thoracic epidural placement.

Methods: A prospective observational study was conducted in 20 patients undergoing elective abdominal and thoracic surgery. The procedure, performed with patients in a lateral position, involved three sequential steps: (1) identification of the interlaminar gap, (2) advancement of the Touhy needle, and (3) identification of the epidural space.
