Subclavian artery pseudoaneurysms (SAPs) are rare and most often secondary to trauma. On the contrary, a mycotic origin is exceedingly rare, and defining this etiology can become challenging. We present a rare case of a tuberculous SAP in a young patient.

The patient was a 21-year-old man with a shadow on a chest roentgenogram taken during a medical checkup. According to blood testing, thoracoabdominal computed tomography, head magnetic resonance imaging, and lung tumor biopsy, we diagnosed a primary retroperitoneal germ cell tumor with multiple lung and brain metastases. Induction chemotherapy (4 courses of Bleomycin, Etoposide and Cisplatin) was started immediately.

Article Synopsis
  • VP shunts are commonly used to treat infants with aqueductal stenosis, but shunt migration can occur, including rare cases of migration into the thoracic cavity due to negative pressure.* -
  • A one-year-old infant with symptoms of raised intracranial pressure underwent emergency VP shunt placement, which initially went well but later resulted in acute breathing issues due to the shunt migrating into the pleural space.* -
  • Imaging showed that the shunt likely tunneled improperly during the first surgery, leading to its eventual migration over three months, necessitating an emergency revision to address the issue.*

Rotational Assessment of Thoracolumbar/Lumbar Curves According to Lowest Instrumented Vertebra Level.

Indian J Orthop

December 2023

Department of Orthopedics and Traumatology, Haydarpasa Numune Education and Research Hospital, Tibbiye Caddesi No:23, Uskudar, 34668 Istanbul, Turkey.

Background: It is not clearly defined in the literature how the lowest instrumented vertebra (LIV) selection effects the rotation of lumbar vertebrae at fused and unfused levels in thoracolumbar/lumbar (TL/L) curves. The aim of this study was to evaluate the rotational profile of structural TL/L curves, corrected with rod derotation manoeuvre, according to LIV level.

Methods: 82 consecutive AIS patients with structural TL/L curves who were treated with long segment posterior instrumentation and fusion were retrospectively evaluated.
