Objective: This study aimed to explore and evaluate a novel method for diagnosing patellar chondromalacia using radiomic features from patellar sagittal T2-weighted images (T2WI).

Introduction: Chondromalacia patella (CMP) is characterized by cartilage degeneration, affects young adults, more women (2:1) and is responsible for 75% of knee pain complaints in the active population. The etiology is multifactorial and may be related to extrinsic factors (trauma and burden) and intrinsic factors (patellar malalignment and quadriceps weakness). Isokinetic dynamometry (ID) can aid in the detection of the causal factors of knee pain related to CMP.

Patellar pathologies are a common cause of knee dysfunction, with Patellofemoral Pain Syndrome (PFPS) alone responsible for 25% of knee-related visits to sports medicine clinics. Non-traumatic conditions, while often overlooked, can also lead to significant discomfort and functional limitations, highlighting the importance of accurate and timely diagnosis for effective management and prevention of complications. This pictorial review examines the radiologic characteristics of various non-traumatic patellar disorders, focusing on imaging modalities such as radiography, computed tomography (CT), and magnetic resonance imaging (MRI).
