Five adult patients underwent penetrating keratoplasty in which corneal tissue from donors 3 months old or younger was used. In each case, a large myopic shift in refraction was found after surgery (mean, 9 diopters). The average postoperative spherical equivalent was -7.35 diopters. This resulted from the extremely steep curvature of the infant donor cornea (mean keratometric value, 57.67 diopters). The induced anisometropia and steep corneal topography of the infant donor cornea prevented visual rehabilitation with spectacles or contact lenses in four of five patients. Our findings suggested that corneal tissue from donors 3 months old or younger may be unsuitable for penetrating keratoplasty in emmetropic adults or anisometropic patients undergoing surgery in the more myopic eye.

Purpose: This report presents a case of fungal keratitis treated with penetrating keratoplasty using a cryopreserved cornea, highlighting the successful maintenance of corneal transparency post infection resolution.
Observations: A 57-year-old man complaining of pain in the right eye was referred to our hospital. Although diagnosed with fungal keratitis, his corneal scraping indicated the presence of , and he was unresponsive to voriconazole, micafungin, and pimaricin treatments.

The aim of this retrospective study was to compare corneal parameters and compliance using a Pentacam HR-Scheimpflug (Pentacam HR) and a swept-source OCT Casia (Casia) in keratoconus (KC) patients post penetrating keratoplasty (PKP) and KC patients without PKP, as well as a control group. Pachymetry measurements were also analyzed using a spectral domain OCT Solix (OCT Solix), Pentacam HR, and Casia. The study included 71 patients (136 keratoconic eyes; group A), 86 eyes with KC post-PKP (group B), 50 eyes with KC without PKP (group C), and 52 control participants (104 eyes).
