Para-sternal echocardiography proved impossible in a case of a large pneumopericardium caused by an oesophageo-pericardial fistula. However, M mode and sector scanning showed separation of the pericardium, giving non-homogeneous echoes, and shaded echographic areas caused by the interference of air which masked all recognisable structures. This occurred in time with systole, a sign which we associate with the pneumopericardium itself.

Pneumopericardium (PPC) is defined by the presence of gas in the pericardial cavity, often leading to cardiac tamponade and a high mortality rate. This report describes a case involving a 33-year-old man found deceased a few meters from a knife, his clothes intact, with no resuscitation attempt made. A knotted scarf was tightly fastened around his neck, without ligature mark.

A nulliparous woman in her late 30s with a history of pericardial patch repair for atrial septal defect and completed treatment for pulmonary tuberculosis 9 years ago presented with chest pain, breathlessness and abdominal pain. Radiological imaging revealed right-sided pneumothorax, pneumopericardium and pneumoperitoneum for which an intercostal drain (ICD) was placed. A contrast-enhanced CT of the abdomen showed a distal stomach perforation, which was managed conservatively.

Pneumopericardium is an unusual condition defined by the presence of gas into the pericardial sac. Gastric perforation into pericardium is a complication of gastric ulcer with high mortality. In this clinical case, we describe the condition of a male with a silent gastric ulcer, which penetrated into the pericardium, developing pneumopericardium and purulent pericarditis as a complication.
