Background: The lower limb venous system, a subject of extensive study due to its high clinical significance on deep vein thrombosis, presents a rare, intriguing, symmetrical variant in this dissection report.
Case Report: We present the uncommon and clinically meaningful bilateral persistent sciatic vein (PSV) variant. The developmental variant was symmetrically detected during a dissection of an 81-year-old female donated cadaver.

Cerclage wiring may be used to optimize the stability of intramedullary nail or plate fixations in comminuted proximal femoral fractures, periprosthetic fractures, and other selected cases. In this article, we presented a novel case of iatrogenic sciatic nerve entrapment from cerclage wiring used to supplement intramedullary nail fixation. We also illustrate and highlight the role of ultrasound in assessing the sciatic nerve to make a timely diagnosis.

Notably, widely adopted methods such as the posterior approach have been linked to elevated rates of posterior hip dislocations and iatrogenic sciatic nerve injuries, while the lateral approach has been associated with superior gluteal nerve injuries. In this study, we propose a refined modification of the McFarland and Osborne approach, aiming to amalgamate the most favorable aspects of prior modifications of the lateral approach to the hip.
