Self-insertion of foreign bodies into the urethra and urinary bladder for autoerotic stimulation is a rather rare urological emergency condition. We present a case of 41-year-old lady who self-inserted a crochet needle into the urethra for sexual pleasure, which was successfully retrieved during examination under general anaesthetic and endoscopic bladder evaluation. Foreign bodies in the urinary bladder are rarely encountered yet represent a urological challenge that requires prompt management in view of possible sequellae if left untreated.

Androgen deprivation therapy (ADT) has been the cornerstone of treatment for advanced prostate cancer for over 65 years. Although there can be worrisome side effects, data will be presented that for men with metastatic prostate cancer, immediate ADT can reduce the likelihood of developing the rare but catastrophic sequellae of metastatic disease, although it is unlikely to prolong survival compared with waiting for symptoms before initiating ADT. The therapeutic armamentarium--physical therapy, drugs, surgery--developed over the last 20 years now gives us the means of providing curative care to nearly all patients with manifestations of neuropathic bladder, meaning that the clinical examination and complementary tests must be conducted under strictly controlled conditions to identify the underlying mechanism. The examination of the perineum almost always reveals a defect when the origin is neurological but is often normal when gynaecologic or urologic factors are involved. Motor command, tonus, reflexes and sensitivity should all be carefully explored to distinguish between central and peripheral causes.

A retrospective survey of total prostatectomies performed for cancer was conducted by 30 urologists members of the French Association of Continuing Education (ANFUC). This survey was limited to the operation itself and perioperative complications observed during hospitalization. Urinary sequellae including incontinence and impotency were excluded and reported elsewhere.
