There is a divergence of opinion concerning the cricopharyngeal muscle defect commonly seen in the pharyngoesophageal area on barium esophagram. Some observers believe this defect is the result of neuromuscular dysfunction with the demonstration of the unrelaxed muscle bundle; however, others believe it is the result of actual hypertrophy of the cricopharyngeal muscle. Radiologic and pathologic study of 24 unselected autopsy cases revealed cricopharyngeal hypertrophy in 13 cases by radiologic criteria. Histologic examination revealed that the cricopharyngeal muscle thickness was uniformly greater in these cases than in the radiographically normal cases. The cricopharyngeal muscle defect is associated with actual hypertrophy of the cricopharyngeal muscle in many cases.

To establish a novel laryngopharyngeal reflux model in Bama minipigs excluding concurrent gastroesophageal reflux through endoscopic cricopharyngeal myotomy. Twelve 8-month-old male Bama minipigs were randomly assigned to three groups: Group 1 underwent cricopharyngeal myotomy alone, Group 2 underwent combined cricopharyngeal and lower esophageal sphincter myotomy, and Group 3 served as the control group. Following a one-week acclimatization period, the respective surgical procedures were performed.

Botulinum toxin injection for retrograde cricopharyngeal muscle dysfunction syndrome.

Eur Ann Otorhinolaryngol Head Neck Dis

December 2024

Service d'ORL, CHU de Hautepierre, Strasbourg, France.

Retrograde cricopharyngeal dysfunction (RCPD) syndrome renders patients unable to belch, causing disabling symptoms that impact quality of life. Injection of botulinum toxin into the cricopharyngeal muscle has been reported as a trial treatment for both therapeutic and diagnostic purposes. We describe the injection technique, under general anesthesia using endoscopy or by transcutaneous injection with electromyographic control.

The belching reflex involves transient lower oesophageal sphincter relaxation and relaxation of the cricopharyngeus (CP). In some individuals, the  latter stage fails, leading to return of gas to the stomach. This pattern is then repeated, which is accompanied by chest pain and loud, gurgling noises.

Article Synopsis
  • Zenker's Diverticulum (ZD) is a common throat condition caused by a herniation of tissue, leading to difficulty swallowing (dysphagia) in most patients.
  • Traditional surgery for ZD has high technical challenges and can result in significant complications, prompting interest in less invasive endoscopic methods.
  • Newer endoscopic techniques like Zenker-POEM and Per Oral Endoscopic Septomyotomy offer alternative treatment options, which are discussed along with their pros, cons, and potential future developments.
