Patients with hyper IgE syndrome (HIES) have varying degree of immunological unbalance that account for deficiency of total T and OKT8 lymphocytes. To investigate the role of immunostimulating drugs in HIES, thymostimulin (TP1) and levamisole were given in highly controlled groups of patients after a wash out period with disodium-chromoglycate (DSCG). Both drugs do not modify clinical picture whereas they do modulate some laboratory parameters as T lymphocyte total number, IgE plasmatic concentration, neutrophil chemotaxis. Furthermore TP1 had a worsening clinical effect in 3 of our patients. In our opinion we feel that the therapies proposed and employed so far for this rare disease must be critically analyzed.

MyD88 deficiency is a rare inborn error of immunity (IEI) characterized by susceptibility to pyogenic infections without overt signs of inflammation. Half of the reported patients belong to Roma descent, an itinerant ethnic group living mostly in Europe, with an increased risk of childhood mortality due to limited access to healthcare services. We describe three unrelated patients from the Campania region in Italy with MyD88 deficiency, all belonging to Roma descent and displaying severe or recurrent infections in early infancy.

Hyperimmunoglobulin E syndrome (HIGES) is a rare immunodeficiency characterized by high levels of immunoglobulin E (IgE) in the setting of various clinical features such as cutaneous candidiasis, asthma, recurrent rashes, and fungal infections. This case describes a 70-year-old male with cachexia and dyspnea found to have a cavitary lesion and aspergilloma, with remarkably high IgE and positive 1,3-β-D-glucan and Aspergillus testing. Herein, we describe the aforementioned case, review the available literature, and hypothesize the connection between invasive fungal infections and HIGES.
