Numerous procedures and techniques are used to correct clawtoe deformity. This paper reports on a new method of correcting clawtoe deformity surgically. Treatment of this condition is not new, and a survey of the literature is offered initially. The theory behind the tendon transposition is discussed and symptoms associated with this deformity are outlined. Variations of this new procedure are listed and illustrated clinically. The authors recommend the peroneus tertius redirection and/or reconstruction procedure as the most adequate way of reducing clawtoe deformity associated with flexible cavus foot.

Hammer toe deformity is a highly prevalent lesser toe deformity and accounts for a high proportion of appointments to foot and ankle clinics. Its etiology is due to extrinsic and intrinsic muscular imbalance, attenuation and subsequent rupture of the plantar plate, and neuromuscular disorders. This leads to marked flexion of the proximal interphalangeal joint and extension of the metatarsophalangeal joint.

Neuropathic deformities of the lesser toes refer to a variety of neuromuscular disorders in which a stiff lesser toe deformity is encountered. Claw toes, hammer toes, and mallet toes can be found. Each deformity is better described as which joint of a particular toe is in a flexed or extended position.

Article Synopsis
  • The claw toe deformity causes pain primarily in adults, and while various treatments exist, none perfectly restore foot mechanics.
  • This review focused on the technical aspects of kinematic evaluation methods for patients with claw toes and examined what is known about correcting the deformity beyond surgical options to reduce fall risks in the elderly.
  • A search of multiple databases revealed various studies on different treatments, but a significant gap exists regarding understanding the postsurgical impact on foot mechanics, highlighting the need for further research in this area.

Article Synopsis
  • A study using finite element analysis was conducted on a new intramedullary implant for correcting claw-hammer toe deformity via proximal interphalangeal joint arthrodesis, which affects 20% of people.
  • The research compared two implant orientations (neutral implant and a 10° angled implant) on a virtual model of a patient’s foot during the toe-off phase of walking, analyzing mechanical stress distributions against a non-surgical foot model.
  • Results showed that the neutral implant significantly reduced tensile and compressive stresses at the proximal phalanx compared to the non-surgical model, while the angled implant increased stress levels, suggesting it could lead to implant failure and is not recommended for use.
