Intracardiac leiomyomatosis is the presence of a benign smooth muscle tumor within a cardiac chamber reaching the heart by direct intravenous extension from an extracardiac source. After a case report and a review of the 13 previously reported cases, the clinical features, diagnostic considerations, differential diagnosis and treatment modalities of intracardiac leiomyomatosis are discussed.

Uterine intravascular leiomyomatosis (IVL) with extension into the right heart is uncommon, with no more than 400 cases reported in the literature since 1907. The present study aims to report three patients with intracardiac IVL surgically treated in our institution, with long-term follow-up. Three female patients in their third to fifth decades of life, with a history of difficult hysterectomy due to extensive myomatosis, presented with symptoms of right-sided heart failure.

Intravascular leiomyomatosis (IVL) is a very rare extension of uterine leiomyoma through the pelvic vessels. It is a benign pathology with malignant potential with a possibility of intra-cardiac extension and metastases (cerebral, pulmonary, lymph node); early diagnosis is very difficult. Prognosis depends on involvement of the inferior vena cava and extension to the right cavities.

Intravenous leiomyomatosis is a rare benign vascular tumor. Intracardiac extension is infrequently but can lead into a threating-life situation. We report a 41-year-old woman who has undergone previous hysterectomy due to uterine myomatosis and now presents with a pelvic mass contacting the venous system through the right internal iliac vein and extending up to the right pulmonary artery.
