Hypertension was found in a 12 year-old Tunisian boy in the weeks following an aortic valvular replacement for aortic regurgitation which was surmised to be rheumatic in origin. In fact, the hypertension and aortic regurgitation were due to the Gröenblad-Strandberg syndrome, or pseudoxanthoma elasticum. The origin of the hypertension was demonstrated by selective renal angiography which showed interruption of the distal cortical renal arteries. Deposition of calcium in the arterial wall was shown by renal ultrasonography. The origin of aortic regurgitation was first imputed to acute rheumatic fever. Thus, the patient presented with complete picture of pseudoxanthoma elasticum with skin, arterial and cardiac involvement. The case reported is the third with a cardiac involvement. The case reported is the third with a cardiac localization of the disease histologically proven in a patient under 20 years of age. The frequency, transmission, histologic findings, etiopathogenesis and clinical aspects of pseudoxanthoma elasticum are reviewed in brief.

We describe a 30-year-old Caribbean-Black woman with a clinical presentation suggestive of a transient ischemic attack (TIA) with no conventional cerebrovascular risk factors, albeit with a newly diagnosed quadricuspid aortic valve (QAV) with moderate aortic regurgitation (AR). Although QAV is a recognized congenital cardiac defect, its association with TIA remains elusive. This case highlights the importance of considering potential atypical etiologies, such as QAV, in the evaluation and management of young patients presenting with cerebrovascular events.

Objectives: To investigate the long-term course of aortic regurgitation (AR) and the width of the proximal ascending aorta (PAA) in patients with ankylosing spondylitis (AS).
Method: This is a follow-up cohort study of patients with AS examined with echocardiography at inclusion (2009 to 2011). Out of the initial 187, a subgroup of 52 patients (54% men, mean age 62 years) was selected for follow-up based on presence/absence of AR at baseline; 26 with AR (18 mild, 7 moderate, 1 severe) and 26 age/sex-matched without AR.

Aortic regurgitation is a common valve disease and can be caused by delineated findings such as fenestrations or hardly discernible alterations of the aortic root geometry. Therefore, aortic regurgitation can be a challenging diagnosis during an autopsy. Cardiac surgeons, however, are confronted with comparable problems during surgery and have developed a refined knowledge of the anatomy of the aortic root including its geometry.
