This review explores the therapeutic potential of the stable gastric pentadecapeptide BPC 157 in addressing electrolyte imbalances, specifically hyperkalemia, hypokalemia, hypermagnesemia, and hyperlithemia. In hyperkalemia, BPC 157 demonstrated a comprehensive counteractive effect against KCl overdose (intraperitoneally, intragastrically, and in vitro), effectively mitigating symptoms such as muscular weakness, hypertension, sphincter dysfunction, arrhythmias, and lethality. It also counteracted the adverse effects of succinylcholine and magnesium overdose, including systemic muscle paralysis, arrhythmias, and hyperkalemia.

Hyperthyroidism is linked to several muscle disorders, including thyrotoxic myopathy, myasthenia gravis, and periodic paralysis. Thyrotoxic periodic paralysis (TPP) is a rare and potentially life-threatening neuromuscular condition that predominantly affects Asian males and is characterized by muscle weakness, hypokalemia, and thyrotoxicosis. Treatment involves potassium supplementation, and beta-blockers.

Severe hypokalemia can primarily present as a weakness of the limbs, without any other clinical manifestation. A life-threatening level of decreased serum potassium level can be unusually present with isolated weakness of the limbs and might be misdiagnosed, or the diagnosis may be delayed.
