Pleuropulmonary involvement secondary to Waldenström's macroglobulinaemia is described from a 70-year-old male. The radiographic abnormalities were unilateral pleural effusion and infiltration of the left lower lobe. There were no respiratory symptoms. After removal of the pleural fluid no other therapy was applied.

Empyema, a type of pleural effusion characterized by pus accumulation in the pleural space, is most often caused by bacterial infections, typically as a complication of pneumonia. This case report presents a 70-year-old man with chronic obstructive pulmonary disease (COPD), rheumatoid arthritis, and chronic bilateral hydropneumothoraces, who developed pyopneumothorax due to dual infections with and . The patient presented with worsening dyspnea, hypoxemia, and respiratory acidosis, requiring hospitalization and chest tube thoracostomy.

Objectives: The aims of this study were to describe the frequency of pleuropulmonary computed tomography (CT) findings in patients with IgG4-related disease (IgG4-RD) and to compare clinical and laboratory characteristics between patients with and without pleuropulmonary involvement in chest CT.
Methods: This is a study conducted within the IgG4-RD study group of the Argentine Society of Rheumatology (GESAR IgG4) cohort of patients with IgG4-RD. Member centers of the group were requested to submit pulmonary CT scans of the patients.

Background: Seropositive rheumatoid arthritis (RA) is associated with significant cardiovascular and pulmonary morbidity. However, screening for early detection of pulmonary involvement especially interstitial lung disease (ILD) is not established in RA.
Methods: We propose a non-invasive radiation-free approach to screen for interstitial lung involvement (ILI) by means of pulmonary function tests (PFT) and pleuro-pulmonary transthoracic ultrasound (LUS) with additional cardiopulmonary exercise tests (CPET) with ECG, and echocardiography.

Rheumatoid arthritis is a chronic inflammatory rheumatic disease that can cause extra-articular manifestations, in particular pleuropulmonary involvement, which is the second leading cause of death in this connective tissue disease. We report a rare pulmonary manifestation of RA. This was a 58-year-old patient being followed for RA on leflunomide with 3 excavated pulmonary nodules measuring 13 mm, 10 mm and 6 mm, for which the diagnostic hypotheses of infection, pulmonary metastases and rheumatoid nodules had been put forward.
